The Illinois Department of Public Health requires specific immunizations for students enrolling in public schools. Students are required to show proof of immunizations including the month/date/year. Failure to provide the information will result in the child’s exclusion from school.
Preschool/Early Childhood
- DPT – four doses
- Polio – 3 doses
- Hib – 1 dose after 15 months (or completed series by 12 months)
- MMR – 1 dose after 12 months
- Varicella – 1 dose after 12 months
- Hepatitis B – 3 doses (third dose after 6 months of age)
- Pneumococcal – Primary series, or proof of 1 dose after 24 months
Kindergarten
- DPT – 4 doses (last dose after 4 years of age)
- Polio – 4 doses (last dose after 4 years of age)
- MMR – 2 doses after 12 months of age
- Varicella – 2 doses after 12 months of age
1st-5th grade
- DPT – 4 doses (last dose after 4 years of age)
- Polio – 3 doses (last dose after 4 years of age) – IPV/OPV mixed will need 4 doses
- MMR – 2 doses after 12 months of age
- Varicella – 2 doses after 12 months of age
6th-8th grade
- Tdap – 1 dose
- Polio – 3 doses (last dose after 4 years of age) – IPV/OPV mixed will need 4 doses
- MMR – 2 doses after 12 months of age
- Varicella – 2 doses after 12 months of age
- Meningococcal – 1 dose on or after 11th birthday.
- Hepatitis B – 3 doses
