Immunization Requirements

The Illinois Department of Public Health requires specific immunizations for students enrolling in public schools. Students are required to show proof of immunizations at the start of the school year.

Preschool/Early Childhood

  • Diphtheria/Tetanus/Pertussis (DTP or DTaP) – four doses
  • Polio – 3 doses
  • Haemophilus influenza type b (Hib) – 1 dose after 15 months (or completed series by 12 months)
  • Measles/Mumps/Rubella (MMR) – 1 dose after 12 months
  • Chicken Pox (varicella) – 1 dose after 12 months
  • Hepatitis B – 3 doses (third dose after 6 months of age)
  • Invasive Pneumococcal Disease (PCV) – Primary series, or proof of 1 dose after 24 months

Kindergarten

  • Diphtheria/Tetanus/Pertussis (DTP or DTaP) – 4 doses (last dose after 4 years of age)
  • Polio – 4 doses (last dose after 4 years of age)
  • Measles/Mumps/Rubella (MMR) – 2 doses after 12 months of age
  • Chicken Pox (varicella) – 2 doses after 12 months of age

1st-5th grade

  • Diphtheria/Tetanus/Pertussis (DTP or DTaP) – 4 doses (last dose after 4 years of age)
  • Polio – 4 doses (last dose after 4 years of age)
  • Measles/Mumps/Rubella (MMR) – 2 doses after 12 months of age
  • Chicken Pox (varicella) – 2 doses after 12 months of age

6th-8th grade

  • Diphtheria/Tetanus/Pertussis (DTP or DTaP) – 1 dose
  • Polio – 4 doses (last dose after 4 years of age)
  • Measles/Mumps/Rubella (MMR) – 2 doses after 12 months of age
  • Chicken Pox (varicella) – 2 doses after 12 months of age
  • Meningococcal – 1 dose on or after 11th birthday.
  • Hepatitis B – 3 doses
