One of the documents you will receive during your registration appointment at schools is information about the Illinois state law requiring families to provide proof that their child has had a health exam and specific immunizations when starting kindergarten.
Children entering kindergarten are required to have the following immunizations.
- Diphtheria/Pertussis/Tetanus (DPT) – 4 doses (last dose after 4 years of age)
- Polio – 4 doses (last dose after 4 years of age)
- Measles/Mumps/Rubella (MMR) – 2 doses after 12 months of age
- Varicella (chicken pox) – 2 doses after 12 months of age
In addition, all children in kindergarten must have proof of an eye exam and dental exam. If you are in need of information regarding health care services in the area, contact your child’s future school. The school nurse can provide you with a list of local organizations.
